Hepatitis B Reactivation in HBsAg Negative Renal Transplant Patients with Evidence of Previous HBV Infection: A Not Neglectable Occurrence

Hepatitis B virus (HBV) infection is frequent among patients with chronic kidney disease (CKD). HBV reactivation after transplant (KT) more common in HBsAg+; however, it can also occur previously infected individuals, particularly those HBsAg negative and total antiHBc positive (HBsAg-/antiHBcT+). However, this population has scarce conflicting data. This study aimed to assess the risk KT recipients previous (antiHBcT+). A retrospective cohort was conducted, including between January 1993 December 2012 HBsAg+ (G1) (antiHBcT+) (G2). of 10,493 transplants were performed period. 203 included (122 81 HBsAg-/anti-HBcT+). The occurred 24.6% (30/38) 9.8% (8/30) G1 G2, respectively. Detectable HCV RNA, indicating coinfection, only variable related antiHBcT+, protecting its occurrence (p: 0.001). In conclusion, infection, occurring approximately 10% cases, should be considered. Complete serology recommended before KT, HBsAg-/anti-HBcT+ monitored procedure. Prophylaxis may indicated when adequate follow-up not feasible. coinfection appears a protective factor for reactivation.
